Intake Process

  1. Email our clinic coordinator to ask for intake paperwork

Prospective patients can self-refer to the clinic by contacting our intake coordinator. You will be sent our electronic intake forms and placed on our treatment waitlist. If you have a preference regarding individual vs group treatment, you are welcome to mention this in the initial email.

2. Complete intake paperwork

Our intake forms consist of demographic information, a detailed intake questionnaire, informed consent documents, and psychological assessments. The paperwork must be completed through our online portal

3. Complete orientation phone call

We will reach out via email to schedule this phone call. The call will include a brief review of your paperwork and a few follow up questions, and will provide you an opportunity to ask any questions that you may have for us.

4. Complete required consultation appointment

Please note that an initial consultation appointment is not a guarantee of treatment. This appointment serves to help us determine whether Ketamine-Assisted Therapy is a safe and appropriate option for you. This appointment is typically 1.5 - 2 hours in length, and includes medical screening, as well as a review of your social and psychological history.

Psychedelic-assisted therapy is an intense and potentially destabilizing process that requires significant psychological and external resources (e.g. stable housing, safe home environment, established supportive relationships). If an applicant does not appear appropriate for treatment at the time that intake paperwork is received, we will do our best to provide information about what steps might lead to appropriateness for treatment at a later date.

We will collaboratively decide on the most optimal arc for your treatment, including the number and frequency of ketamine and integration sessions, individual versus group treatment, and dosing and routes of administration of ketamine.  In some cases, we may request one or more preparatory sessions to further discuss your history and support preparation for your first Ketamine session.
